Referral Information

At YOUNIQUE Care we are dedicated to providing high quality person-centered, supported living and live in care in London and UK wide for adults and the elderly with Autism, complex needs, including mental health difficulties, physical and/or learning disabilities. When someone has been referred to us, we will work together with them and their loved ones to create a tailored care package that will promote their health and wellbeing. Our carers will be able to provide supported living services within London and the rest of the UK during the day and/or night.

Referral Criteria

To complete a referral the service user must live within the UK and be over the age of 18. All referrals need to be made with the express consent of the service user.

Referral Process

Upon referral, the service user will receive a visit from one of our team where their needs will be assessed prior to care provision. During this consultation our team will be able to get a full understanding of the service user's condition, what they need assistance with and any other important information. We will also liaise with loved ones and medical professionals so we have a full understanding of what level of supported living care is needed.

We will then be able to discuss the client's goals which will enable us to create a structured care package that will enable them to fulfill these. We will listen carefully to the service user's preferences so that our care will reflect these. Once care is delivered, there will be opportunities for evaluation and amendments, so that the way we provide our services can be modified and adapted efficiently, as per service user's desires and demands.
